Please Note: All of our clinics have transitioned to a new Electronic Medical Records system and we now use a different patient portal called Elation Passport. All patient medical records have been securely transferred over to the new system for your convenience. In order to actively use the new patient portal (Elation Passport) an invitation must be sent from your clinic staff. To receive the invitation please call your clinic or ask for the invitation to be sent after your next appointment.
